Afterthoughts on EVAC2020

Today the use of antibiotics is under attack because of the wrong usage in the past. Antibiotics are not to be used anymore at a low dosage as growth promotor. Instead, they are to be used as VMPs at full power following the SPCs and technical pharmaceutical guidelines to cure sick animals and to safeguard One Health.

The 2020 edition focused on the highly debated molecule Colistin and the danger of too strict regulations.

The key 5 disclosures:

  • At a curative dosage, there is no acquisition of resistance for the water-soluble veterinary medicinal product of colistin.
  • The use of colistin, at a curative dosage, does not select for MCR-gene bearing strains
  • Colistin is essential for veterinary medicine as a treatment in a group through drinking water
  • Colistin resistance in human medicine is very low and better alternatives are coming on the market
  • To ban colistin as a molecule to treat animals at a therapeutic dosage will increase AMR in the short run and will have a detrimental impact on One Health in the long run.

New research from the Royal Veterinary College in London demonstrated that chickens treated orally with the Veterinary Medicin Product (VMP) of Colistin through drinking water at a therapeutic dosage of 150.000 IU/kg live body weight obtained steady-state levels within the 12 hours after the start of the treatment. The mean intestinal concentration at day 2 after treatment was 8.6 mg/ kg and this far exceeds the MIC of E.coli harbouring MCR-genes. This method of treatment does not select for resistance, can be given in a short period and is environment and animal friendly, safeguarding the One Health principle and the judicious usage of antibiotics: as less as possible, as much as needed.
